    Nice piece. Reminds me of Crichton's "Airframe." Ever since I read that, the phrase "cascading sequence of failures" has been lodged in my memory banks. This situation seems like a classic example.

    This was an interesting note to me:

    I’m reminded that none of those things were complete when they left the factory—because their builders realized they didn’t have to be complete. The job could be done at any time in the future with a software update.
    I believe he's talking about the agile software development methodology, which has been a darling of the development world for quite a while. Release, review, patch, release...and repeat indefinitely. It's certainly a decent way to create computer games, life enhancing applications, and operating systems. But for something that holds the lives of 100s of passengers in its proper operation...it's darned near criminal.
